LA| Louisiana Bulletin 2026-08 announces that the Department of Insurance has immediately updated the Louisiana Standardized Credentialing Application on its website to stay aligned with NCQA requirements, adding a practitioner language field in the demographic section, removing prepopulated responses from the General Questions section, and revising the attestation to a single 120‑day signature validity period without extra attestation date fields; health insurance issuers and HMOs must now use this updated version, available under Regulatory Forms in Form and Rate Filings, and may direct questions to the Office of Health, Life, and Annuity at LDI.
