Illinois Protects Vaccine Access: Emergency Rule Requires Comprehensive Insurance Coverage for All Recommended Immunizations

IL| The Illinois Department of Insurance issued Company Bulletin 2025-18 in response to recent changes in federal vaccine recommendations, to ensure continued health insurance coverage for preventive immunizations in the state. This directive, effective immediately, requires all Illinois-regulated health insurance plans—including self-funded plans for state and local government employees—to cover, without cost-sharing, all vaccines recommended either by the CDC’s Advisory Committee on Immunization Practices (ACIP) as of August 7, 2025, or according to separate guidance from the Illinois Department of Public Health (IDPH) if the federal recommendations become less rigorous. The bulletin underscores state-level authority to restore more comprehensive coverage when federal policies change, insists that insurance policy forms reflect this urgent compliance, and clarifies that these requirements apply broadly except for certain grandfathered or excepted benefit plans. It also describes logistical steps for insurers needing to amend their policy forms and directs questions to the Department, highlighting the urgent public health context and the legal authority under which the rule is promulgated.
