Louisiana Issues Directive 225 Regarding Coverage for Fertility Preservation

LA| Directive 225, issued by the Louisiana Department of Insurance, outlines new requirements for health insurance coverage of fertility preservation services. Effective January 31, 2025, this directive mandates that all health insurance issuers, health maintenance organizations, and self-insurers in Louisiana must comply with La. R.S. 22:1036.11. This statute requires insurers to provide coverage for medically necessary standard fertility preservation services, particularly for individuals diagnosed with cancer whose treatment may cause iatrogenic infertility. 

Key points:

  • Coverage must include all standard fertility preservation services and associated health care services, including prescription drugs, supplies, and drug therapy.
  • Preauthorization is not required for coverage of standard fertility preservation services.
  • “Standard fertility preservation services” are defined as oocyte and sperm preservation procedures consistent with guidelines from the American Society of Clinical Oncology or the American Society for Reproductive Medicine.
  • Insurers must provide immediate coverage for these services, including all related health care services and treatments.
