MD| The Maryland Insurance Administration’s Bulletin 26-24, issued October 15, 2026, announces adoption of a uniform Continuity of Care Request Form under Maryland Insurance Article § 15-112(y)(4). Carriers, providers, and health systems must use the form for applicable requests to continue receiving care after a provider’s network participation terminates, including requests under § 15-112(b) or 42 U.S.C. § 300gg-113. The bulletin establishes the following requirements and limitations:
- Required use covers qualifying transitional-care requests involving primary care providers, including primary dental care providers, and behavioral health providers, subject to the specified prior-claim requirements. It also covers qualifying continuing-care patients, including those receiving treatment for serious and complex conditions, inpatient care, nonelective surgery and related postoperative care, pregnancy, or terminal illness.
- Electronic versions are permitted if they retain all instructions and information from the required template and reproduce the language in each field verbatim.
- Stand-alone dental carriers using provider panels must use the form for transitional-care requests involving primary dental care providers; use for specialty dental providers is optional. The bulletin does not specify a separate implementation deadline.
