MI| Michigan DIFS Bulletin 2026-01-INS announces that the department has updated its auto insurance utilization review forms for clarity, including the certification application, provider appeal request, insurer reply, insurer annual report, and provider attestation forms, which are now posted on the DIFS utilization review webpage. As of February 15, 2026, DIFS will no longer accept outdated versions, and providers and insurers are expected to update their processes and communications to use only the revised forms.
