AZ| The Surplus Line Association of AZ announced on June 6, 2025 – 2025A reports will be available starting July 7, 2025.
By August 15, 2025, you must report transactions to SLA, and file reports and pay premium tax to the Department of Insurance, as follows:
- Only file, report and pay tax on transactions for which Arizona is the home state.
- File transactions and run the 2025A report for insurance procured between January 1 through June 30, 2025.
- All reports must be submitted through OPTins (www.optins.org). We automatically email an Excel version of the tax report to you EACH TIME you run your tax report from the “Report” tab. – The PDF version of the tax report is only for your records – it can not be used to file your report.
